When the / bookmark push failed (oxygen#12), the client printed only the server's raw io: text. The hub log had a bare 500 with no path, bookmark or cause, and finding the bug took a local oxy serve and reading the source (metalgear feedback item 2).

Refusals should name the reference and the failing step. The server should log refusals and failed requests with repository and operation, and storage failures should be 500s.
