Closes issue #148, for oxygenhub#329. Backend::push_notes(name, report, headers) -> Vec<(reference, note)>, called before after_push: each note is added to that accepted reference's result, which oxy push already prints in place of "updated", so no client change is needed. Default: no notes. Test: a_host_adds_a_note_to_an_accepted_reference. Gate: fmt, clippy -D warnings, tests all pass.
